Default Rabies vaccine and hypodermic needles

I am going to Thailand, and was thinking I might also go to vietnam via cambodia. But i was wondering about rabies vaccines and how important it is. I have discussed this with people who have been to thailand and they said it is not needed for thailand because there is good access to medical care If you are Infected.

But Is this something I need to get for cambodia and vietnam? How common Is rabies?/how likely am i to encounter some rabbid dog? and how easy is it to get good medical help if I am bitten. I dont intend to stray too far from any towns into the wild but I would like to see some less tourist infested areas.
I read somewhere online that I might need to go all the way back to thailand If I get bitten in cambodia? It seems a little extreme to me, but Is this true?

My Doctor said It would be a good Idea, but that opinion would be based on official goverment guidelines.

My doctor also said I should bring some hypodermic needles as part of a first aid kit. Are needles worth bringing? Are local Medical centers really so bad they dont have clean needles?

I get the impresion my doctor is being overly cautious, but I would appreciate some first hand experience on these issues.
