I have done a similar trip and you absolutely must fly BKK - Chang Mai - BKK - Phuket - BKK since the trains are far too slow and your schedule is so tight, but doable. If you are flying United or Thai Air get Star Alliance's Thailand Airpass before you leave the US. Your other choice is a Bangkok Air Airpass.
The first choice is Thai Air and the second is Bangkok Air. If you can't get one of these airpasses go for a regular ticket from Thai Air. It is not worth the risk of late/delayed/cancelled flights if you go with other carriers.
